Basic Information

Please have this information ready:
- Installation Address
- Meter Number
- Customer Contact Information
- Receipt or contractor invoice
- Mailing address for where to send the incentive check if approved
- W-9 if your business will be receiving the incentive check
Heat Pump Water Heater Incentive
- Manufacturer and model number of new equipment
- Photos of the old electric water heater and the newly-installed heat pump water heater, including:
  - A wide shot showing the unit in its surrounding area, and
  - A close up of the equipment nameplate.

Heat Pump Projects Require Pre-Authorization: Has pre-authorization already been granted for this project? Great! If not, please find the form on our website or call us at (509) 661-8008 before filling out this application.
- Photos of all new system indoor and outdoor units, including:
  - A wide shot showing the equipment and its surrounding area, and
  - A close up of the equipment nameplate.
- New equipment specifications including efficiency ratings and size
- AHRI certificate(s)
Insulation Incentive
- Before and after photos of the insulation project, including:
  - A wide shot showing the project space, and
  - A close up with a ruler showing insulation depth.
Lighting Incentive
- Completed lighting calculator found here, Lighting Calculator Excel workbook (Excel), and also available on our website at chelanpud.org/lighting.
- Please note: At this time, we are not able to give initial estimates for lighting projects as part of this application process.
Thermostat Incentive
- Manufacturer, model, and serial number for new thermostat(s)
- Photo of the installed thermostat, including:
  - A wide shot showing the installed thermostat in its new location, and
  - A close up of the thermostat.
Windows Incentive
- Proof of U-Factor rating (on receipt, invoice, or photo of the NFRC window sticker)
- Before and after photos of the window project, including:
  - A wide shot showing the window and the surrounding area, and
  - A close up clearly showing the old single-pane, single-pane storm window, or double-pane window type being replaced.
